Assessment for Support

The range of services we provide allows people to get the help they need while maintaining their independence and quality of life. All support packages are designed around the individual. Building a successful placement depends on getting to know the person through a detailed assessment process and then developing a service around the person’s needs.

We begin by working out a personalised plan of support that is designed, in partnership, with the client, our support team, and anyone else the client would like to involve, such as their family, social worker, or advocate. Centricorp Care will then provide a support package that is tailor-made to meet the client’s individual needs. The package of support remains flexible and will be reviewed and adapted as many times as is needed to meet the client’s changing needs.

Before we commence any service, we always undertake an initial assessment. This usually involves meeting the individual and their family, talking to the person’s social worker, or responsible healthcare professionals. This helps us to understand if we can meet the needs of the person in terms of having the knowledge and skills to support the person and the ability to manage potential risks. We would also explore how much support is required and the practicalities surrounding this. After this initial assessment stage, we are able to advise the client if we can meet the needs of the client.

A full assessment will then commence, which will look in detail at the social, health, and functional needs of the individual and how this would be supported and indeed funded. The information from the assessments forms the basis of the individual’s Person Centred Plan (PCP). This is a comprehensive document that details what is important to a person right now and for the future and what support the person needs. On a more day-to-day level, the PCP will be used to construct activity support plans and the associated structures and resources to achieve the goals in the PCP.
